A Joint Training Programme on the Principles and Practice of Care Management and the role of the Care Coordinator for Health and Social Care Staff who will undertake this Role
FOUR-DAY PROGRAMME:
Day One: Key Concepts in CPA/Care Management
Aims:
- To identify the expectations of participants and the issues which they wish to address during the programme.
- To review the legal and policy context in which the role of care coordinator is set.
- To provide a detailed outline of the role of care coordinator and review the principles of CPA/care management.
- To review the importance of valuing diversity including the diverse value systems of differing professions
Day Two:The CPA/Care Management Process:
Aims:
- To identify the eligibility criteria and their implications for practice and interdisciplinary working
- To review the ways in which different professionals currently provide screening
- To review the implications of the Human Rights Act
- To review principles and practice in relation to risk assessment
- To outline the principles of duty
Day Three: The Assessment Process
Aims:
- To review the key practice elements involved in the assessment process, including problem solving; helping skills; interview skills; communication skills; focusing and goal setting.
Day Four: Care Planning and Care Management
Aims:
- To review practice in relation to care planning and care management
- To provide a detailed briefing on the role of the care coordinator in care planning and care management
- To review practice in relation to client support
- To review practice in relation to making referrals to other agencies
- To discuss practice in relation to monitoring and reviews.
- To provide an opportunity for participants to work on strategies to overcome the problems identified at the outset of the course in the light of what has been learnt
To provide an opportunity to establish the criteria for best practice in the role of care coordinator.
